APPLICATION FOR DIPLOMA CERTIFICATE

Applications are called from eligible students who have completed the Higher National Diploma programme by the academic year 2019 to issue a Diploma Certificate.

Please submit the Google Form Application

DC-1 Form

By submitting, A copy of your responses will be emailed to the address you provided.

Print your application form with email body And the DC-1 Form

Affix the bank slip which you paid to the Peoples Bank & place your signature on the DC -1 form as requested

Applicants are advised to submit the following documents along with the application form

1. Certified copy of the Detailed Certificate issued by the institute
2. Certified copy of the in-plant training certificate / letter confirming the completion of the internship
3. Certified copy of applicant's birth certificate
4. An affidavit if the name mentioned in the result sheets is different from the name in the application.
5. If applicable, documents related to student transfers
6. Photocopies of all the pages of the student record book
7. Duly filled DC -1 Form with original signature

A duly completed hard copy of the application form & attached documents should be forwarded to the Registrar (Registrar, Advanced Technological Institute, Bandaranayake Avenue, Kegalle) of the ATI - Kegalle by hand or registered post
